Fax Order Form


Our shopping cart is secure using the highest level of encryption.  We prefer that you use our shopping cart because we do not have to re-key your data, which slows down order fulfillment.  But some people would prefer to fax in their order.  For those folks, we provide a fax order form.  Our fax order form comes in two flavors:

Adobe Acrobat Fax Order Form which you can fill out by typing on your own computer screen, printing out and faxing to us at 520-750-1442.   This form is very convenient for you to fill out - just type into its fields.  It is easy for us to read, and it does not transmit your information over the web.  To use this order form, you have to have Adobe Acrobat or the Adobe Acrobat reader installed on your computer.  If you want to install the free Adobe Acrobat reader, here is the link to the Adobe website, which provides a free download.

Plain order form which you fill out by hand.  This blank form will open in Wordpad, Microsoft Word or Word Perfect.  Print it out, fill it in, and fax it to us at 520-750-1442.
